Open the app, tap your name in the top right, scroll to the bottom of Your Account and choose Delete my account. You'll be asked for your password and to type DELETE.
It happens straight away. There is no waiting period and no way to undo it.
Open the appEmail alfred@groundrushinc.com from any address and tell us the username or the mobile number on the account. We'll confirm it's you and delete it within 7 days.

Group totals — the number of push-ups the whole challenge has done — are not reduced, because they're a count of work, not a record about you.
Profiles you added under your login are deleted with it. They have no password and no phone of their own, so nobody could sign in to reach them afterwards. If you only want to remove one of them, do it from Your Account in the app instead — that leaves your own account alone.
